Secure Forms

Secure Forms is a secure WordPress environment intended to house web forms at Boise State University.

Gravity Forms is the WordPress plugin used for web form development and management via the Secure Forms site at https://secureforms.boisestate.edu/

Each WordPress site at Boise State has a corresponding Secure Forms site. E.g., sspa.boisestate.edu has a Secure Forms site at secureforms.boisestate.edu/sspa.

The production site is what the general public will see, and the Secure Forms site is where web forms are created. Once a form has been created in the Secure Forms environment it needs to be linked to the department’s production site.

How To

Log in to both your production site and secure forms site.

In your production site create a ‘Thank You’ page, then copy the URL for your ‘Thank You’ page.

The ‘Thank You’ landing page is where the user will be redirected to once they have submitted their responses. We’ll create this page first because we will need the URL once your form is created.

In your Secure Forms site, create a new form. Select ‘Edit.’

Choose ‘Confirmation’ and select ‘Redirect.’

In the field below ‘Redirect’ paste in the URL from your ‘Thank You’ page which was created above. This will redirect users back to your production site once they click on ‘submit’ at the bottom of the form. Now you are ready to add fields to your form. Once you have created your form be sure to save it.

Next create a page in Secure Forms to house your newly created form. In your Secure Forms site go to ‘Pages’ and select ‘Add New.’ Give the new page an appropriate title (maybe similar to the name of your form).

Now insert your form into the newly created page: click on the ‘Add Gravity Form’ square icon just to the right of ‘Upload/Insert’ and select your form from the drop down box and click ‘Insert Form’. Save your page.

Finally, create a link on your production site that will take the user to your form.
